Dr. Goodnight knew he wanted to be a plastic surgeon since he started medical school. He chose the training that offered him the most experience in facial plastic surgery as this area has the greatest concentration of technically demanding cosmetic surgeries. Thus Dr. Goodnight did his Head and Neck Surgery training at UCLA Medical Center where he performed hundreds of facial cosmetic & econstructive surgeries. Dr. Goodnight has been tightening skin with the CO2 laser for decades and he now recommends the fractional CO2 laser since you heal more quickly. He is one of the few facial plastic surgoens to be trained in the techniques to rejuvenate your skin outside and in.

SmartSkin is a fractional carbon dioxide (C02) laser. Like other fractional laser skin resurfacing devices, it works by splitting the laser’s light into tiny beams that are each about one-tenth the size of a hair shaft. The light penetrates the skin to a controlled depth, where its heat vaporizes the targeted cells, while leaving the tiny areas around each tiny beam of light untouched. Only about 20 to 30 percent of the targeted area is actually treated, yet the entire area benefits from the process and eventually becomes rejuvenated. Since less skin is actually damaged by the laser, the recovery time is generally shorter than with non-fractional C02 lasers.

Resurfaces and Rejuvenates; SmartSkin is an ablati ve laser, which means it removes the outermost top layer of the epidermis, the stratum corneum. The heat from the laser light also reaches the dermis, however, where it remodels the skin by stimulating the formation of new collagen, the protein that gives skin youthful, healthy-looking volume. The loss of skin volume (usually as the result of sun damage) is what causes lines and wrinkles to form.

What It Treats: SmartSkin helps minimize fine lines and wrinkles and promotes a more even skin tone and texture. It also helps resolve other skin problems associated with aging, such as solar lentigines (better known as age spots), keratoses (tiny raised bumps on the skin), and wide pores. In addition, the SmartSkin laser has been found to be effective in reducing the appearance of acne scars.

How Long Are Treatments? Treatments generally take about 20 minutes. You can opt for a single aggressive treatment session or several less-aggressive ones that are spaced over several weeks. After the procedure you’ll feel,and look, as if you’ve had a sunburn. The more aggressive the treatment, the more redness and swelling you’ll experience -and the more time you’ll need to recover. Most people feel comfortable returning to their normal activities within 4 day s or so.

Rejuvenate Yourself Outside and In

For the past 20 years longevity physicians have been prescribing “bio-identical” hormone replacement therapy in an attempt to delay the effects of biological aging. Everything degenerates as we get older. When the degeneration occurs in our critical endocrine organs, the levels of our hormones diminish. A decrease in the production of hormones begins in middle age and continues in a linear fashion until old age. As the levels of hormones decline, so do we decline both physically and mentally. Replacing all the deficient hormones – thyroid, testosterone, estrogen, progesterone, DHEA and growth hormone- to levels that we had in our youth has been touted by anti-aging physicians to be beneficial to both our health and quality of life.

It is touted to strengthen and bolster our bodies as well as our minds. Hormone replacement is recommended to change the precipitous decline after middle age into a gradual transition that will be less noticeable.There are thousands of studies on hormone treatment for adults. The vast majority are for true deficiencies such as hypothyroidism, hypogonadism, menopause and adult growth hormone deficiency. Relatively few studies have evaluated the effects of replacement in healthy adults for the purpose of anti-aging. I am not aware of any studies that have evaluated the simultaneous restoration of all hormones to youthful levels. There have been small 6 month studies revealing improvements from growth hormone alone. Rudman et al found a 8.8% increases in lean body mass, 14.4 % decrease in adipose mass, 1.6% increase in lumbar vertebral bone density and a 7.1% increase in skin thickness in 12 healthy elderly men who initially had low normal growth hormone levels. There have been 6 month studies with sex hormones plus growth hormone showing decreased fat mass, and increased LBM. There have been several studies for replenishing thyroid hormones for patients with normal thyroid hormone levels demonstrating benefit. There have been relatively few studies on “bio-identical” estrogen and progesterone replacement (as opposed to synthetic) for menopausal women. The natural hormone studies have shown benefit while the large synthetic Prempro studies have raised concerns that risks may outweigh the benefits. “Bio-identical” refers to the hormone being an exact replica of the hormone your body naturally produces. Since the hormones were meant to work in synergy, the optimal results would be expected when all hormones are replaced.

A review of the touted biological effects for anti-aging of each hormone should help you understand why all hormones should be restored to optimal levels. The benefits listed are essentially the biologic function of each of the hormones. Their effects are clearly demonstrated in the literature. The theory is that as the levels of these hormones drop, despite being in the normal range for ones age, their function gets performed to a progressively lower extent. When the hormones are restored to their youthful levels they can restore biological function back to their youthful levels. Unfortunately the more deterioration that has occurred of the target organs the less youthful the restoration if the target organ deterioration is irreversible. “Bio-identical” HRT is thus more of a preventive measure of further irreversible deterioration. Dr. Goodnight performs your facelift using tumescent local anesthesia. The face is made numb similar to methods used with liposuction. The procedure is actually begun with the liposuction cannula doing all the initial dissection. Dr. Goodnight prefers the Smartlipo liposuction cannula as it seals the blood vessels prior to the incisions. With the blood vessels sealed, bruising is minimal and healing is rapid. Results are still long lasting since the underlying muscle and fascia is also tightened. Having had a facial fat transfer 3 months prior, under local anesthesia, the youthful fullness of her face is restored. Sure, you would accept a scar in the back of your head for a great head of hair if there was no other option. But, with Neograft, thereis an option.

The FUE (follicular unit extraction) hair transplant is minimally invasive procedure. With FUE each graft from the back of the head (donor area) is harvested individually and placed in the thinning or balding areas on the top of the head. The hair at the back of the head is genetically programmed not to fall out (not affected by the DHT hormone). The NeoGraft medical device is designed to help the doctor offer his patients this latest, most advanced procedure in hair transplantation while overcoming the problems with manual FUE and those of the “strip” method.

With the FUE method there is little bleeding and fewer complications than with the “strip” method.
